From Miami to Baghdad to Porto Alegre, the three surgeons quickly discovered that while their languages and landscapes differ, their challenges are remarkably similar. Across all regions, unqualified providers are performing genital filler injections — often with dangerous, permanent substances like silicone or PMMA (bone cement).
Dr. Hasan shared his perspective from Iraq, explaining how his training led him to choose hyaluronic acid (HA) as the safest and most reversible option:
“I started using HA fillers because they’re natural, reversible, and can be done under local anesthesia. I wanted to learn from the best, and that’s why I came to Florida.”

Dr. Veber noted that in Brazil, silicone and PMMA injections are officially banned yet still used illegally by non-medical injectors such as pharmacists and dentists.

“Not ten miles from us, someone is injecting silicone right now — and potentially changing a man’s life forever. Once it’s in, it’s almost impossible to remove without degloving the penis.”
The consensus: run from anything permanent. Silicone, PMMA, and fat transfers have caused life-altering complications from infections to penile deformities.

When it comes to male enhancement, anatomical expertise matters. Dr. Hasan and Dr. Veber both emphasized that prosthetic urologists, surgeons who routinely perform penile implant procedures are uniquely qualified to perform HA injections safely.
“We understand every structure of the penis — where to inject and where to avoid,” said Dr. Hasan.
“Most of the patients we see now are trying to fix problems caused by untrained injectors.”
Dr. Perito agreed:
“These are the guys who live and breathe andrology. They’ve held hundreds of prostheses in their hands and know the anatomy inside and out. That’s who you want working on your penis.”
